Motor Vehicle Report Request

As part of our hiring background and investigation, we may obtain consumer report information, including a Motor Vehicle Report. Under the provisions of the Fair Credit Reporting Act (15 USC at 1681-1681u) as amended, before we can seek such reports, we must have your written permission to obtain the information.

AUTHORIZATION AND RELEASE TO OBTAIN INFORMATION

Under the provisions of the Fair Credit Reporting Act, 15 USC, Section 1681 et seq., the Americans with Disabilities Act and all applicable federal, state and local laws, I hereby authorize and permit Stewart Stainless Supply, Inc. to obtain a Motor Vehicle Report.

• I understand that Stewart Stainless Supply, Inc. will submit the following information to their insurance agency, Insurance Office of America (IOA).
• I understand that IOA will request the Motor Vehicle Report for the purpose of determining insurability.
• I understand that IOA may share the information contained in the Motor Vehicle Report with Stewart Stainless Supply, Inc.’s current insurance carrier or prospective insurance carriers.
• I understand that the Motor Vehicle Report or its’ contents will not be shared with or disclosed to Stewart Stainless Supply, Inc.
